5 steps for CYO Winter Sports Registration at St. Joseph - Avon Lake

 

CYO Winter Registration is open from September 6 to October 2
Late Registration Dates: October 3 to October 14
Registration Fees for Winter 2011 Sports (LATE REGISTRATION FEE - ADD $25):
Boys and Girls Basketball (Grade 3-8) $120.00
Boys and Girls Basketball (Grade 9-12) $110.00
Cheerleading (Grade 3-8) $55.00


Also Print, Fill out, and turn into St. Joseph's CYO Drop Box by gym doors.
-Current physical form (Good for 13 Months)
-Emergency Medical Authorization Form
-Eligibility Request Form(ERF) (Signed by players from other parishes)

A COUPLE OF IMPORTANT POINTS TO KEEP IN MIND AS YOU COMPLETE THE REGISTRATION PROCESS: Please fill out ALL of the fields in the form as completely as possible - including athlete eligibility, parish, school and physical date.
